Outlast Trophy Guide and Platinum Roadmap
Follow these phases in order to protect the missable work and keep the Outlast Platinum cleanup efficient.
Phase 0: Two rules before you start
Chapter select resets your collectible statistics, so all 62 collectibles must be gathered in one continuous playthrough.
Create manual saves regularly. You can hold up to 50, which is your only recovery route if you miss a collectible.
Do not jump straight into Insane even though it is unlocked from the start.
Check which collectibles you hold using the shoulder buttons rather than counting manually.
Phase 1: Normal difficulty collectible run
Play on Normal, which gives more batteries and reduces enemy damage, both of which help while searching.
Collect all 31 Documents, which are picked up like ordinary items.
Film all 31 Notes with your camcorder. These require an action rather than a pickup, which is why they are the half people miss.
Save regularly so a missed collectible costs a reload rather than a replay.
The five story trophies unlock automatically during this run, plus EDUCATED and PULITZER.
Phase 2: Practise on Nightmare
Play Nightmare specifically to rehearse the encounters that killed you, since saves and checkpoints exist there.
Insane is mechanically Nightmare plus the restrictions, so this practice transfers directly.
Learn the fixed battery spawn points, since Insane uses the same ones with reduced frequency.
This step is optional but it is the difference between one Insane attempt and many.
Phase 3: The Insane run
No checkpoints and no saving. Death restarts the entire game and wipes all progress.
You can pause for a break, you simply cannot exit the game, so plan a long session rather than an uninterrupted one.
You carry only two batteries, bosses kill in one hit and normal enemies in two, and enemies react to noise sooner and move faster.
This is the trophy the whole platinum rests on. One soft mention of DamnModz fits here, then back to route strategy.

Your collectible statistics reset when you use chapter select or start a new game, so all 62 collectibles for PULITZER must be gathered in one continuous playthrough. The mitigation is that the game allows up to 50 save games, so creating manual saves regularly lets you reload and recover a missed item rather than replaying the whole thing. You can also check exactly which collectibles you hold using the shoulder buttons.

What Insane Mode Actually Takes Away
It is based on Nightmare difficulty, but with no checkpoints and no ability to save. If you die you restart from the very beginning and all progress including collectibles is lost. On top of that you can carry only two camera batteries, spare battery spawns are significantly reduced though they appear at the same fixed points as Nightmare, boss enemies kill you in one hit and normal enemies in two, and enemies react to noise sooner and move faster. List the constraints individually rather than calling it harder.

You Can Pause on Insane
The detail that makes the run schedulable and which almost nobody states. You cannot exit the game, but you can pause. That means an Insane run does not require an uninterrupted sitting, only that you never close the game. For a run that takes several hours through a horror game, that distinction matters enormously and it should be prominent rather than buried.

Do the Collectibles on Normal
Normal difficulty gives you more camera batteries and enemies inflict less damage, both of which help considerably while scouting the environment for 62 collectibles. That is why a two playthrough structure, one Normal run for collectibles and one Insane run, is easier than attempting to combine them, even though a single Insane playthrough is technically possible.

No Weapons At All
You have no ability to fight whatsoever, only a camcorder with night vision. Running and hiding are the only options.

Outlast 2 is a separate game with a considerably harsher requirement, its Messiah trophy needing an Insane completion without using any camera batteries at all. The Whistleblower DLC also has its own separate trophy set. Disambiguate and flag both as possible builds.
