Follow these phases in order to protect the missable work and keep the Darksiders 3 Platinum cleanup efficient.
Phase 0: Set up your backups and your plan
Save corruption is a documented risk. Back up regularly, alternating between different USB devices and PS Plus Cloud storage.
Save to USB before every boss fight via the console's Application Saved Data Management.
Decide your route. New Game Plus with selectable difficulty means you can do a comfortable 100 percent run then a Apocalyptic speedrun, which is far easier than one Apocalyptic playthrough.
Use the classic control scheme, which makes counter timing and healing noticeably easier.
Phase 1: A comfortable 100 percent run
Play on a lower difficulty and focus on completing everything, including the collectibles, the Crucible and the Void.
Reach level 50 as early as you can for Big Spender and the combat advantage. Put every point into Strength and Health, keeping Health a few points behind.
Get the Fafnir Enhancement from Usiel after defeating Lust. It reflects 100 percent of incoming damage back at enemies when you are hit.
Farm souls at Crossroads in Haven, killing everything then using the game options kill yourself command to respawn them.
Take the three missables here, Demon Meddler by killing Abraxis in the Skar area after the Stasis Hollow, For Whom the Bell Tolls by ignoring one of the two bosses in that fight, and A Parting Gift.
Phase 2: Apocalyptic via New Game Plus
After the final boss, choose to go to Armageddon Mode and set the difficulty to Apocalyptic.
All your gear and levels carry over, which is why this is a speedrun rather than a struggle.
You can also earn the DLC Armageddon trophy in this same run alongside the base Death Incarnate.
This is the run buyers most often want handled if they are attempting Apocalyptic cold. One soft mention of DamnModz fits here.
Phase 3: Level 100 and cleanup
Reach level 100, which may require a further New Game Plus run. One player needed New Game Plus Plus for the final seven levels.
Finish the Crucible if you have not, since your main game progress carries into it and it is best done when strong.
Clean up any remaining miscellaneous trophies using the New Game Plus run.
Check the current glitch position before assuming a trophy is permanently lost, since many were reported.

New Game Plus Makes Apocalyptic Easy
First section after the Quick Info box. Apocalyptic is mandatory, which makes this list look like a 5 to 7 out of 10. A later patch added New Game Plus, and a further patch added the option to choose your difficulty at the start of it. So the correct plan is a comfortable 100 percent playthrough on a lower difficulty, then a speedrun on Apocalyptic via New Game Plus with all your gear and levels intact. That takes the difficulty down to roughly a 3. A player also confirms you can earn both the base Death Incarnate and the DLC Armageddon trophy in that single New Game Plus run.

Back Up Your Saves, Seriously
Save corruption is a documented risk on this game. The recommended routine is regular backups alternating between different USB devices and PS Plus Cloud storage, plus saving to USB before each boss fight via the console's Application Saved Data Management. On a 35 hour game with a mandatory hardest-difficulty run, losing a save is the worst outcome available, and no page leads with this.

The Three Missables
Demon Meddler, For Whom the Bell Tolls and A Parting Gift. For Whom the Bell Tolls depends on ignoring one of two bosses during a specific fight. Demon Meddler requires killing Abraxis in the Skar area after receiving the Stasis Hollow. All three can also be lost through dialogue choices during their respective quests, since two are presented as story quests that are easy to ignore entirely.
Efficient route
Reach Level 50 Early
It grants Big Spender and a substantial combat advantage that makes everything afterwards easier. Put every point into Strength and Health only, keeping Health a few points behind Strength. Souls are plentiful, so as long as you are not buying consumables recklessly you can get every trophy in one playthrough.
The Fafnir Enhancement
Obtained from Usiel after defeating Lust. It deflects 100 percent of incoming damage back at enemies whenever you are hit, which is an enormous advantage on a mandatory Apocalyptic run. Flag it as a priority pickup rather than an optional extra.
The Crossroads Soul Farm
Teleport to Crossroads in Haven, kill every enemy there, then open the menu, go to game options and use the kill yourself command. That respawns the enemies for another pass. Very convenient early on when levelling matters most.
Use the Classic Control Scheme
Community advice, it makes the timing for counters much easier and healing more responsive. On a soulslike with mandatory hardest difficulty, control feel is worth a line.

The Version 1.00 Exploit
On an unpatched physical disc you can earn the Apocalyptic trophy while playing on normal difficulty. It requires disconnecting from the internet before installing and never reconnecting, or the game gets stuck after Maker's Forge demanding the patch, and it only works on disc since digital downloads the patch automatically. Note that this conflicts with the New Game Plus route, since New Game Plus is itself a patch addition.
