Follow these phases in order to protect the missable work and keep the Blasphemous Platinum cleanup efficient.
Phase 0: Understand the ending order
Ending B must be obtained before you complete the seventh arena battle. Finishing all seven evolves your Thorn into the Cvstodia of Sin, which permanently locks Ending B out.
Ending A is the true ending and requires the Cvstodia of Sin, so the order is forced rather than optional.
None of the three free content updates add trophies, so ignore them entirely for the platinum unless you want the story.
Deliver Medicinal Herbs to Tirso before defeating area bosses. That single habit protects a chain of missable items.
Phase 1: First playthrough, 100 percent and Ending B
Aim for 100 percent game completion for Witness of the Miracle, which fails if you miss any missable item.
Handle Gemino's Olive Tree quest and the Kissers of Wounds quest in Albero carefully. These are where the platinum is most often lost.
Collect everything as you go, the rosary beads, prayers, relics, sword hearts and the corpses to talk to.
Grind Tears of Atonement so you can buy every ability and the Candelaria shop items.
Take Ending B before finishing the seventh arena.
Phase 2: The arenas and Ending A
Complete all seven arena battles for Detestatio Sacrorum. Your Thorn evolves into the Cvstodia of Sin.
Beat the final boss with the Cvstodia of Sin in your inventory for Ending A, the true ending, which also unlocks the Son of the Miracle outfit.
With both endings banked in one run, you avoid a second full playthrough for the Cleofas trophies.
Phase 3: Requiem Aeternam and the speedrun
Defeat every boss without using a Bile Flask for Requiem Aeternam, the hardest trophy on the list.
This can be spread across your first run or saved for a knowledgeable second run, whichever suits.
Pass through the bronze door on the Bridge of Calvary in under three hours, which pairs naturally with a second run.
This is the skill wall of the platinum. One soft mention of DamnModz fits here, then back to boss strategy.
Phase 4: The Cleofas trophies and the platinum
Only two trophies genuinely cannot share a playthrough, both tied to the Cleofas side quest.
Having taken both endings in your first run, you only need a partial second run for these.
That is what makes the total roughly 2.5 playthroughs rather than four.

First section after the Quick Info box. Completing all seven arena battles evolves your Thorn into the Cvstodia of Sin, and holding that item permanently locks you out of Ending B. Ending A is the true ending and requires the Cvstodia of Sin, so the order is forced. Take Ending B first, then finish the arenas and take Ending A. Getting this wrong costs an entire extra playthrough.

None of the DLC Adds Trophies
The Stir of Dawn, Strife and Ruin and Wounds of Eventide are all free content updates adding areas, bosses, items, NPCs and even a new canon ending, and none of them add a single trophy. All 45 non-platinum trophies are base game only. That is a large reduction in perceived scope and it deserves stating plainly.

Witness of the Miracle requires 100 percent game completion and fails if you miss any missable item. The two places that actually catch people are Gemino's Olive Tree quest and the Kissers of Wounds quest in Albero, both of which can lead to permanently missing items if you handle them carelessly. Give both step by step.

Deliver them before defeating area bosses. It is a simple rule that protects a chain of missable items and it is easy to overlook while pushing forward.
Efficient route
Requiem Aeternam
Defeat every boss without using a Bile Flask. The hardest trophy on the list and the one worth its own strategy section, including which bosses to attempt on which run. It can be left for a second playthrough if you would rather enjoy the first.
The Speedrun Trophy
Pass through the bronze door on the Bridge of Calvary in under three hours. Note it pairs naturally with a knowledgeable second run rather than a first playthrough.
Grind Tears of Atonement Early
You need enough to buy every ability and the Candelaria shop items. The first playthrough is where that accumulates, so do not skip enemies unnecessarily.
The Wounds of Eventide Interaction
No trophy depends on it, but Esdras is now linked to the DLC and meeting him on the bridge voids your access to it. Ending C, the true canon ending, requires transforming the Immaculate Bead into the Weight of True Guilt and clearing all Confessor Dungeons before defeating Escribar.
Not Blasphemous II
The sequel is a separate game with its own list, its own missables and a documented invincibility glitch. Disambiguate and consider it as a separate build.
