Follow these phases in order to protect the missable work and keep the Kingdom Come: Deliverance Platinum cleanup efficient.
Phase 0: Three rules before you start
Do not learn to read yet. Wait until From the Ashes has rebuilt Pribyslavitz and Trial and Error has popped.
Keep your Drinking skill below level 13, or you will lose the blackout drunk trophy in A Woman's Lot.
Start collecting herbs immediately. You need 10,000 and it is the largest cumulative requirement in the game.
Decide now whether you are going for Merciful and Virgin, since both are restraint trophies that shape how you play from hour one.
Phase 1: Main story with restraint
Play the main questline without killing anyone except Runt for Merciful, the rarest trophy at 0.6 percent.
Stay celibate for Virgin, and use the per-quest autosaves to pick up Casanova and King Charming separately.
During A Rock and a Hard Place, share the quarry money and keep Matthew and Fritz alive. They train Stealth and Unarmed and feed a From the Ashes trophy later.
The game autosaves at the start of every main quest, so a missed quest trophy is recoverable without a new playthrough.
Phase 2: Side quests and the missables
Work through the side quests, watching the 12 named missables as you go.
Kingdom Come, the main completion trophy, needs 80 of the 105 quests finished.
If you own the DLC, expect Completionist to pop early since it is counter-based.
Use the four speech books to jump your speech skill rather than grinding conversations.
Phase 3: The cumulative grind
Finish the 10,000 herbs, the money and skill requirements and the remaining miscellaneous trophies.
This is roughly a third of the total time on its own, which is why the herbs should have been started in hour one.

Check the current status of Freud and Judas before assuming either is broken or fixed.
Phase 4: The DLC sets, optional
None of the four DLC sets are required for the base platinum.
Read the DLC notes before starting them, since several base game quests affect DLC trophies and you do not want a second playthrough.
A Woman's Lot carries its own missables including the blackout drunk trophy that the Drinking level rule protects.
If you want another platinum, the Japanese version has a separate list that stacks.

Two Skills That Silently Kill Trophies
First section after the Quick Info box. Do not learn to read until after the From the Ashes DLC quest A Place to Call Home and Pribyslavitz is rebuilt, and until Trial and Error has popped. And keep your Drinking skill below level 13, because a Woman's Lot quest requires getting blackout drunk and at level 13 you no longer black out at all, making that trophy unobtainable. Both are skills a player raises through completely ordinary play, and neither warning appears in a mainstream guide body.

One Playthrough Is Enough
The game keeps an autosave from the start of every main quest, which functions as a chapter select for every quest-related trophy. Combined with reloading if you kill or romance someone by mistake, that means a 150 hour list only needs one run. Say this early, because 12 missables on a game this long reads as multiple playthroughs.

The Four Book Speech Shortcut
Four books boost the speech skill dramatically, available from the Rattay innkeeper, a charlatan in Sasau, a beggar via random encounter, and Treasure 15. One player reports going from speech 17 to 20 in under two minutes using them. This turns the Bard trophy from a grind into a short errand and it currently exists only in a comment thread.
Efficient route
Merciful and Virgin, the Two Restraint Trophies
Merciful requires killing nobody in the main questline except Runt, at 0.6 percent, which is the hardest trophy on the list. Virgin requires completing the entire game celibate, which conflicts with Casanova and King Charming.
Keep Matthew and Fritz Alive
During A Rock and a Hard Place you can share the quarry money or keep it and beat them up. Keeping them alive matters, because Matthew trains Stealth and Fritz trains Unarmed, and both can be sent to Pribyslavitz with the From the Ashes DLC for a silver trophy. An early side quest choice affecting a trophy 100 hours later.
Start the 10,000 Herbs Immediately
Collect herbs as you pass them from the first hour rather than treating it as an end game grind. It is the single largest cumulative requirement.
Completionist Pops Early With DLC
If you own the DLC, Completionist unlocks well before you finish all base game quests, because it is counter-based rather than tied to a specific list.
